Does Medicare Pay for Geriatric Care Management?

Understanding Medicare, Pennsylvania Aging Services, and Private Care Management

Medicare covers certain care coordination services, while Pennsylvania offers programs that help older adults access care and community support. Private geriatric care management provides another option for families seeking personalized, hands-on assistance.

Understanding these options can help you find the right support for yourself or someone you love.

Understanding Your Care Management Options

Care management services vary depending on who provides them, how they're accessed, and the type of assistance needed. These options can complement one another, and families may benefit from more than one.

Sunset Senior Concierge:
Bridging the Gaps
in Care

We’re there when you can’t be.

Even when older adults have an established healthcare team, community resources, or family support, everyday needs and medical responsibilities can be difficult to manage, particularly when loved ones are balancing work, family obligations, or distance.

Sunset Senior Concierge provides personalized geriatric care management and senior concierge services that complement existing support systems, helping older adults maintain independence while giving families added peace of mind.

How We Help

  • Geriatric Care Management: Comprehensive assessments, individualized care planning, coordination among healthcare providers, and ongoing support.

  • Medical Escort & Advocacy: Transportation to and from medical appointments as part of care management, with in-person participation, informed questions, clarification of diagnoses and treatment recommendations, and real-time professional advocacy.

  • Care Coordination & Follow-Through: Scheduling follow-up appointments, communicating recommendations, and helping ensure important next steps are completed.

  • Senior Concierge Services: Assistance with errands, pharmacy pickups, shopping, and other everyday needs.

  • Daily Living Support: Assistance with instrumental activities of daily living (IADLs), including managing appointments and personal schedules, meal preparation, household organization and downsizing, and participation in community activities.

  • Hospital & Family Support: In-person hospital advocacy when available, care transition assistance, and updates to authorized family members.

Support in Action.

Follow Up

We don't simply help arrange services. When appropriate, we're there in person, accompanying clients to appointments, participating in discussions with the provider and client, helping arrange next steps, and sharing relevant health information with their care team and authorized family members, with the client’s permission.

By developing a comprehensive understanding of each client’s health history, medical conditions, and care goals, our licensed clinical social worker can ask informed questions, recognize when additional clarification may be needed, and help connect information across healthcare providers.

Our goal is to work alongside existing caregivers, healthcare professionals, and community programs, providing additional support when care needs extend beyond available services or when family caregivers cannot be there in person.

Professional guidance. Personalized support. Peace of mind.
