How We Support Families Navigating Dementia

Caring for a loved one experiencing memory loss can feel overwhelming and emotionally complex. We provide guidance, education, and care coordination to support families navigating dementia and related cognitive changes.

Support May Include:

  • Caregiver guidance and education

  • Planning transitions to higher levels of care

  • Referrals to trusteed community resources

  • Assistance navigating Medicare, Medicaid, and VA systems

  • Respite coordination support

  • Family consultation and planning meetings


Families often find that dementia support extends beyond education and planning to include thoughtful care coordination, reliable transportation to medical appointments, and practical day-to-day errand support.

Our approach is supportive and educational in nature. We do not provide diagnosis or medical treatment but work alongside healthcare providers to promote dignity, safety, and informed decision-making.

Dementia consultation services are available remotely throughout Pennsylvania, with in-person support currently available in Florida.
