What Is Geriatric Care Management?

Care management begins with a comprehensive assessment and individualized care plan, followed by ongoing coordination, advocacy, and clinical oversight as needs evolve.

Care management complements - but does not replace - medical or home health services.

Care management consultations are available remotely throughout Pennsylvania. In-person services are currently available in Florida.

When Care Management Is Most Valuable

Care management is especially valuable when:

  • Multiple providers are involved in care

  • Following hospital discharge or major medical events

  • Medication regimens become complex

  • Adult children live out of state or work full-time

  • Safety concerns arise in the home

  • Cognitive changes or dementia symptoms appear
