Does Medicare Pay for Geriatric Care Managers?
Understanding Medicare, Pennsylvania Aging Services, and Private Care Management
Medicare covers certain care coordination services, while Pennsylvania offers programs that help older adults access care and community support. Private geriatric care management provides another option for families seeking personalized, hands-on assistance.
Understanding these options can help you find the right support for yourself or someone you love.
Understanding Your Care Management Options
Care management services vary depending on who provides them, how they're accessed, and the type of assistance needed. These options can complement one another, and families may benefit from more than one.

Care management can look different depending on the provider or program. Medicare and state-supported care managers often focus on assessing needs, developing care plans, coordinating services, and monitoring progress.
Private geriatric care management may offer additional flexibility and direct involvement, depending on the provider. At Sunset Senior Concierge, that includes personally accompanying clients to medical appointments, real-time care advocacy, attending and participating in medical visits, ensuring continuity from one appointment to the next, and other senior concierge services, as appropriate.

Follow Up
We don't simply help arrange services. When appropriate, we're there in person, accompanying clients to appointments, participating in discussions with the provider and client, helping arrange next steps, and sharing relevant health information with their care team and authorized family members, with the client’s permission.
By developing a comprehensive understanding of each client’s health history, medical conditions, and care goals, our licensed clinical social worker can ask informed questions, recognize when additional clarification may be needed, and help connect information across healthcare providers.
Our goal is to work alongside existing caregivers, healthcare professionals, and community programs, providing additional support when care needs extend beyond available services or when family caregivers cannot be there in person.
Professional guidance. Personalized support. Peace of mind.

Medicare &
Medicare Advantage:
What Care Management Services Are Covered?
Medicare covers certain care coordination and management services designed to help eligible beneficiaries manage chronic conditions, navigate healthcare, and transition between care settings.
Original Medicare (Parts A & B)
Medicare Part B covers certain services provided through participating healthcare professionals, including:
Chronic Care Management: Care planning, medication reviews, and coordination for eligible individuals with multiple chronic conditions.
Care Navigation: Help understanding complex medical conditions, accessing resources, and coordinating care.
Transitional Care Management: Follow-up and coordination after discharge from a hospital or certain other healthcare settings.
These services are generally arranged through your physician or healthcare team rather than by enrolling directly through Medicare. Eligibility requirements and out-of-pocket costs may apply.
Medicare Advantage (Part C)
Medicare Advantage plans may offer care management with nurse case managers, care navigation, and additional support services. Availability and access vary by plan, individual needs, and eligibility.
How Are These Services Provided?
Care management services under Original Medicare and Medicare Advantage are generally coordinated through healthcare providers, clinical teams, or health plans. Much of the ongoing communication and follow-up may occur by telephone or electronically, although some services involve in-person care during regularly scheduled appointments.
Transportation and appointment accompaniment:
Original Medicare generally does not cover routine transportation to medical appointments or a dedicated advocate who attends visits with you.
Some Medicare Advantage plans may offer transportation benefits, but availability, eligibility, and covered services vary by plan. Transportation benefits do not necessarily include in-room medical advocacy.
Ask your healthcare provider or Medicare Advantage plan what services are available.
What About Veterans?
Eligible veterans may also have access to care coordination, geriatric assessments, home-based services, transportation reimbursement, and caregiver support through the Department of Veterans Affairs (VA). Eligibility and service availability vary.
Veterans can contact their VA PACT healthcare team or social worker to explore available services.

Pennsylvania Aging Services:
Care Management & Community Support
The Pennsylvania Department of Aging works with local Area Agencies on Aging (AAAs) to help older adults access services that support independence, safety, and quality of life.
Adults age 60 and older may qualify for a range of community-based services, depending on their needs, circumstances, and program eligibility.
What Services May Be Available?
Care Management: Assessments, individualized care planning, coordination of services, and ongoing monitoring.
In-Home Support: Assistance with eligible household tasks, personal care needs, and other services that support independent living.
Meals & Transportation: Home-delivered meals and access to public or shared-ride transportation programs, such as Chester County's Chesco Connect. These services help older adults reach appointments and community destinations, but generally do not include a dedicated medical escort who attends appointments and provides advocacy.
Caregiver Support: Resources, education, and potential financial assistance or respite services for eligible family caregivers.
Health & Wellness: Connections to health promotion programs, benefits counseling, and other community resources.
How Do You Access These Services?
Contact your local Area Agency on Aging to discuss available programs and request an assessment or referral.
Pennsylvania's OPTIONS program provides eligible adults age 60 and older with access to care management and certain home- and community-based services. There is no income limit to apply, although financial contributions may be required based on income and program guidelines.
Services are based on assessed needs and program availability, and not every individual will qualify for every service.
