Sunset Senior Concierge is a supportive option for older adults who want help without giving up their independence. We work closely with individuals and families to create personalized, flexible plans that adapt as needs change over time. From coordinating care and scheduling appointments, to transportation and simple errands, we are here to help when you need us.

Our Services
Care Management
Services may include:
-Manage & organize medical appointments
-Support informed decision-making
-Assistance w/creation of online electronic health portals
-Health communication between primary and speciality clinics
-Ensure continuity of care, and promoting safety, independence, and dignity
Services begin with an individualized assessment and care plan, followed by ongoing coordination of care, advocacy, and monitoring as needs evolve and does not replace medical or home-health care.
We work collaboratively with healthcare providers, community resources, and family members to reduce fragmentation of care-while easing strain on families.
Transportation & Errand Support
Non-emergent transport may include:
-Medical appointments (waiting with & assisting your loved one during their appointment
-Pharmacy & essential outings
-Grocery shopping
-Personal care (beauty, hair grooming)
-Exercise & social engagements
Reliable, non-medical assistance to help older adults stay connected and independent.
When appropriate, transportation services may complement broader care coordination efforts with family members or care teams.
Transportation and errand support is non-medical.
Dementia & Family Support
Support may include:
-Caregiver guidance
-Planning for transitions to higher levels of care
-Referrals to community resources
-Assistance navigating systems such as, Medicare, Medicaid, and VA-related services
-Respite
Our goal is to help families feel informed, supported, and less overwhelmed—while honoring the dignity and preferences of the individual experiencing dementia.
Services are supportive and educational in nature and do not include diagnosis, treatment, or medical decision-making.

Hello. I’m Natasha Hilts, the founder of Sunset Senior Concierge and the person you’ll be speaking with during your consultation.
I’m a Licensed Clinical Social Worker (LCSW) and Certified Dementia Care Specialist (CDCS) with 15+ years of experience in public service across military, Federal, and community-based healthcare systems. My background includes active military service in the U.S. Air Force, where I served as a mental health clinician & Officer in Charge of the Family Advocacy Program; civilian mental health clinical work supporting the military & the United States Secret Service; and, geriatric care coordination within the Department of Veterans Affairs.
I work closely with older adults and their families as they navigate complex care decisions, cognitive changes, and life transitions. My approach is thoughtful, practical, and relationship-driven.
